Gliomas are diffuse and invasive brain tumors with the nefarious ability to evade even seemingly draconian treatment measures. Here we introduce a simple mathematical model for drug delivery of chemotherapeutic agents to treat such a tumor. The model predicts that heterogeneity in drug delivery related to variability in vascular density throughout the brain results in an apparent tumor reduction based on imaging studies despite continual spread beyond the resolution of the imaging modality. We discuss a clinical example for which the (...) model-predicted scenario is relevant. The analysis and results suggest an explanation for the clinical problem of the long-standing confounding observation of shrinkage of the lesion in certain areas of the brain with continued growth in other areas. We present a multiscale integrationist interpretation of the boundaries of cognitive systems, using the Markov blanket formalism of the variational free energy principle. This interpretation is intended as a corrective for the philosophical debate over internalist and externalist interpretations of cognitive boundaries; we stake out a compromise position. We first survey key principles of new radical views of cognition. We then describe an internalist interpretation premised on the Markov blanket formalism. Having reviewed these accounts, we develop our positive multiscale account. (...) We argue that the statistical seclusion of internal from external states of the system—entailed by the existence of a Markov boundary—can coexist happily with the multiscale integration of the system through its dynamics. Our approach does not privilege any given boundary, nor does it argue that all boundaries are equally prescient. We argue that the relevant boundaries of cognition depend on the level being characterised and the explanatory interests that guide investigation. We approach the issue of how and where to draw the boundaries of cognitive systems through a multiscale ontology of cognitive systems, which offers a multidisciplinary research heuristic for cognitive science. Scientific realism has been advanced as an interpretation of the natural sciences but never the behavioral sciences. This book introduces a novel version of scientific realism, Measured Realism, that characterizes the kind of theoretical progress in the social and psychological sciences that is uneven but indisputable. It proposes a theory of measurement, Population-Guided Estimation, that connects natural, psychological, and social scientific inquiry. Presenting quantitative methods in the behavioral sciences as at once successful and regulated by the world, the book will (...) engage philosophers of science, historians of science, sociologists of science, and scientists interested in the foundations of their own disciplines. Part I introduces measured realism beginning with a novel realist theory of measurement (Population-Guided Estimation, or PGE). It examines two other versions of scientific realism — minimal (Hacking) and robust (Boyd, Putnam, and Newton-Smith) — and advances measured realism as best supported by the evidence in the behavioral sciences. It also critically evaluates the anti-realist arguments of van Fraassen, Laudan, and Fine. Part II formulates and develops a version of epistemological naturalism most appropriate to measured realism. Drawing upon the realist theory of measurement developed earlier, it favorably appraises a variety of statistical methods in the behavioral sciences against selected criticisms. It concludes that traditional, narrative methods in the behavioral and social sciences are unreliable by the measures of epistemological naturalism. (shrink)

Background and objectives Conscientious refusal of abortion has been discussed widely by medical ethicists but little information on practitioners' opinions exists. The American College of Obstetricians and Gynecologists (ACOG) issued recommendations about conscientious refusal. We used a vignette experiment to examine obstetrician-gynecologists' (OB/GYN) support for the recommendations. Design A national survey of OB/GYN physicians contained a vignette experiment in which an OB/GYN doctor refused a requested elective abortion. The vignette varied two issues recently addressed by the ACOG ethics committee—whether the (...) doctor referred and whether the doctor disclosed their objection to the abortion. Participants and setting 1800 OB/GYN randomly selected physicians were asked to complete a mail survey containing the vignette. The response rate was 66% (n=1154) after excluding 40 ineligible cases. Measurement Physicians indicated their approval for the vignette doctor's decision. Main results Overall, 43% of OB/GYN physicians responded that the conscientious refusal exercised by the vignette physician was appropriate. 70% rated the vignette doctor as acting appropriately when a referral was made. This dropped to 51% when the doctor disclosed objections to the patient, and to 12% when the doctor disclosed objections and refused to make a referral. Consistent with previous research, males were more likely to support disclosure and refusal to refer. Highly religious physicians supported non-referral but not disclosure. Conclusion OB/GYN physicians are less likely to support conscientious refusal of abortion if physicians disclose their objections to patients. This is at odds with ACOG recommendations and with some models of the doctor–patient relationship. (shrink)

Science is not taken for granted any longer. Society, politics and the media pose critical questions tending to censorship or at least control of science. How does science respond? It cannot exist and develop without freedom, but this does not mean freedom to amass knowledge and apply technological applications at any price and without restrictions. Science should be autonomous, but is not value-free. A distintion is made between external and internal social/ethical problems. The former refer to questions of the social/ethical (...) context and consequences of scientific research, and the latter to the rules of ‘good practice’ and scientific integrity. The role of academies of science, and of associations of such academies (e.g. All European Academies (ALLEA)) in developing codes of good scientific practice and fostering a proper sense of scientific values and standards is further discussed. Il semble aussi que ce qui d’ordinaire tient lieu de réponse à cette question dans l’esprit des lecteurs de Descartes, c’est le sentiment vague qu’il s’agit là de la première vérité qui surgisse après la dévastation laissée par le doute universel de la Première Méditation, ou peut-être qui surgisse (...) au cœur de ce doute, puisque le doute ne s’autodétruit que pour faire place à ego sumOr s’en tenir à cela pour justifier le statut de principe métaphysique du cogito, c’est confondre priorité chronologique et primauté métaphysique ou épistémique: en effet, qu’y aurait-il dans le seul fait de se présenter ainsi qui assure que le cogito doive fonder d’autres connaissances, d’éventuelles sciences? Qu’est-ce qui empêcherait aussi qu’il ne soit lui-même assujetti à d’autres connaissances, plus fondamentales, et dont le méditant ne se serait peut-être pas encore aperçu? Il est vrai que cette confusion est compréhensible, puisque la primauté métaphysique du cogito coïncide effectivement avec sa priorité dans l’ordre des connaissancesMais dans le doute ambiant, celle-ci n’est en réalité qu’un effet de celle-là, et le simple fait de constater l’une ne revient pas à être assuré de l’autre. Il ne peut s’agir donc que d’une coïncidence, et la question se pose dès lors de savoir comment se traduit sa primauté, c’est-à-dire comment doivent se concevoir ses rapports avec le reste de nos éventuelles connaissances. We consider the potential impact of Computer Supported Cooperative Work, with special reference to large technically advanced projects involving several organizations. It is vital that such projects are managed efficiently, without delays, since a product that reaches the market a few months earlier than its competitors enjoys a great advantage. Traditional methods of coordinating large projects, based on hierarchical communication, tend to produce delays, since technicians at remote sites are obliged to solve coordination problems by passing them up the hierachy. (...) It would be better if such problems were solved by improvising conferences among the technicians; Computer Supported Cooperative Work will provide the technical means of implementing this heterarchical style of management without losing control of the project. The use of computers as a social medium raises methodological and ethical issues which are discussed in the final section. (shrink)
